Since December 2006, through the CECAP program, a total of 9,365 women have been screened, of which 232 were found to be VIA positive. 122 underwent cryotherapy and the rest were scheduled for treatment in a follow-up visit or referred to partner tertiary institution for further diagnosis and management.
Site performance is assessed by conducting on site or virtual monitoring which is regularly done by the CECAP Management Team (CMT).
Trained providers are evaluated individually through several assessment tools to facilitate transfer of learning. This would ensure that the knowledge, skills and attitudes learned during training are practiced and improved with the goal of attaining skill proficiency.
Monitoring and evaluation are carried out by the CMT to assess whether program indicators are being met and to evaluate the extent to which the program is having the desired impact. This is instrumental in identifying problems and formulating solutions to improve the program.
Training
The Clinical Skills Training Course is a 7-day course which is designed to prepare each participant to be competent in performing VIA either as a means of screening for cervical cancer or its precursors or in preparation for treatment of the abnormal cervix by cryotherapy. The CST is led by certified CECAP trainers, who have underwent the CST themselves as well as the Training of Trainers which have equipped them with the knowledge and the skills to conduct an SVA workshop.
The training course consists of a variety of training techniques which includes lectures, role-playing, simulators, demonstration, observation and hands-on training, among others. This ensures that the participants will acquire the necessary knowledge, attitude and skills for quality service delivery.
The training conducted by CECAP is truly one of its kind for the Mastery Leaning Approach is adapted which assumes that all participants can learn the required knowledge, attitudes and skills provided there is sufficient time and appropriate methods are used.
The philosophy underlying the mastery learning approach is one of a continual assessment of participant learning where the clinical trainer regularly informs participants of their progress in learning new information and skills.
The 4 pillars of the mastery learning approach include:
1. Adult Learning: Learning is participatory, relevant and practical. Training is built on what the participant already knows or has experienced and provides opportunities for practicing new skills.
2. Behavior Modeling: Learning by observation takes place in three stages. In the first stage, the participant performs the procedure and acquires a mental picture of the required steps. Once the mental image is acquired, the participant attempts to perform the procedure, usually with supervision. Next, the participant practices until skill competency is achieved and s/he feels confident performing the procedure. The final stage, skill proficiency, only occurs with repeated practice over time.
3. Competency-based Training: This is learning by doing. To successfully accomplish CBT, the clinical skill or activity to be taught first must be broken down into its essential steps. Each step is then analyzed to determine the most efficient and safe way to perform and learn it. This process is called standardization. Competency based learning guides and evaluation checklists have been developed to facilitate learning among participants. The clinical trainer is always present as s/he coaches and provides timely and objective feedback to the participant before, during and after practice.
4. Humanistic Training Techniques: The use of anatomic models, which closely simulate the human body is employed. Only when skill competency and some degree of skill proficiency have been demonstrated should participants have their first contact with an actual client.
To qualify for training, a participant must be:
• a licensed physician, nurse or midwife currently and actively providing reproductive health services in the community health centers.
• must belong to the proposed service delivery point or referral center.
• must be committed to the program’s vision and mission.
Trained Clinical Providers
The program has successfully trained 89 health professionals. CECAP was able to hone technical experts serving as local champions in advocating for cervical cancer prevention. The majority of those trained have become dedicated service providers who deliver the “single visit approach” effectively in the community. Some are now acting as clinical supervisors, ensuring quality assurance of services provided and managing referrals of patients that require further diagnosis and management.

Trainers
Last January 7 to 12, 2008, CECAP conducted a Training of Trainers involving 14 health professionals previously trained on the Single Visit Approach for the purpose of preparing trainers to conduct competency based clinical training for service providers of the VIA and cryotherapy. This was facilitated by Dr. Enriquito Lu, Director of Family Planning, Reproductive Health and Cervical Cancer Prevention Division of Jhpiego, An Affiliate of Johns Hopkins University.

Barangay Health Workers
In the Philippines, it is critical to work with the barangay health workers (BHWs) who are the primary catalysts in mobilizing community members to be informed and actively participate in health programs. Hence, CECAP held 4 BHW orientation workshops were a total of seventy three (73) BHWs participated. They were all given the CECAP flipchart, an important visual and information tool which they can use during house calls to the women in their community. Likewise, they were given a comprehensive lecture on cervical cancer and were taught effective communication and presentation skills which will aid them as they counsel women to seek screening.
